Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

• List of current prescriptions and/or over-the-counter medication, including dose and frequency
• Pertinent information about your medical and surgical history
• Any recent x-rays or appropriate records you may have
• Completed patient forms

At the start of your first office visit, you will be asked to complete our Notice of Patients Privacy Rights and Release of Medical Information form. Please arrive early to allow time to complete this form. Or you may print and complete the form at home prior to your visit.

Insurance and Payment Information

Dr. Hanna participates with and accepts the following insurance carriers and payment sources:

• Medicare with supplement
• Most PPO insurances
• Motion picture Insurance
• HMO (Health Care Partners and facey) Must have Prior Authorization

Note to patient: It is the patient’s responsibility to verify that their health insurance is in network with Dr. Ahed Hanna, prior to treatment.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
